Adding your facility to the Guide: please complete all applicable fields in the appropriate Survey Form. If the facility has more than one space, you must complete a separate form for each space (e.g. Facility= Herberger Theatre; Space= Stage West). Be sure to submit each form when completed.

NOTE: The form(s) must be completed and submitted in one sitting (it cannot be saved and submitted later). For that reason we suggest that you first print out the appropriate Survey Form(s) and collect all the requested information before entering the data online.

Updating your information: Once your facility is listed in the Guide, you can update information by simply filling in the appropriate field that needs updating and submit the form.

Tip: Review the entries of similar facilities in the Cultural Facility Resource Directory for content and style continuity.
