Affirmative Action Requirements
All firms wanting to do business with the city must have Affirmative Action (AA) forms on file with the Equal Opportunity Department. The forms can be downloaded in Microsoft Word at http://phoenix.gov/ftpalias/payf/attachb.doc. It also is available in Adobe Acrobat (PDF) format at http://phoenix.gov/ftpalias/payf/attachb.pdf, which allows the document to be downloaded, viewed and printed with all of its original formatting and graphics. To view files in this format, first download a copy of the Adobe Acrobat Reader and follow the instructions for installation.

If you have 15 or more workers, complete parts A, B and C. You will be eligible to do business with the city for one year from the time your paperwork is received.

If you have less than 15 workers, complete part D only. You will be eligible to do business with the city for two years from the time your paperwork is received.
