How to become certified as a Minority- Woman-owned, Small and/or Disadvantaged Business Enterprise with the city of Phoenix.Minority, Woman, Disadvantaged and Small Business Enterprise Online Access
The city of Phoenix has a program that certifies minority- and woman-owned and small businesses for participation in special city incentive procurement and contracting programs.
Please read the online brochure that provides a brief overview of these certification programs. If you wish to apply for certification in these programs, print out and complete the Uniform Certification Application for New Applicants. Return the application, the Personal Financial Statement, the Personal Financial Statement Affidavit, the Affidavit of Certification, and supporting documentation to the city at the address listed on the application.
Although business certification is valid for three years, you are required to submit an Annual Update Application along a No Change Affidavit, the Personal Financial Statement, the Personal Financial Statement Affidavit, and your previous year's personal and business taxes every year to maintain your certification status.
As a certified business owner with the city of Phoenix, you are also required to renew your certification status every three years. To recertify your firm, please print out and complete the Recertification Application and submit it along with the Personal Financial Statement, Personal Financial Statement Affidavit, the Affidavit of Certification, and the Affidavit of No Change with your personal and business taxes.
In addition, Phoenix requires all companies, certified or not certified, to submit paperwork regarding the firm's affirmative action employment efforts. We encourage you to read the Affirmative Action Instructions. Before any firm can do business with Phoenix, application forms must be completed and on file with the city's Equal Opportunity Department. If you have 14 or fewer employees, complete part D only.
