Request for Proposals for Executive Physical Examination Services

The city of Phoenix Personnel Department is soliciting proposals from qualified physicians or physician groups to provide medical physical examinations to eligible executive-level staff. This RFP seeks a provider of services for the period of Oct. 1, 2008, through Sept. 30, 2011, with an option to renew for two additional one-year periods in accordance with the specifications and provisions contained in the Request for Proposals (RFP) packet.

The city's initial program for executive physicals was implemented in 1977. The number of executive physical exams varies each year; however, the annual range is approximately 75-100 physical exams. Please note that city executives have the option of using their personal physician for the exam.

The RFP packet can be downloaded in the formats specified below. Second Ave., Phoenix, AZ 85003.

The deadline for submission is 3 p.m. on Friday, August 8th, 2008.
