2008 Community Recognition Awards

The Human Services Department provides comprehensive social services to improve the quality of life for Phoenix residents.

Nominations currently are being accepted by the city of Phoenix Human Services Department for the Community Recognition Awards Breakfast on Thursday, Dec. 11, 2008 at the Arizona Biltmore.

ELIGIBILITY

Any individuals, organizations or agencies who have contributed outstanding volunteerism or service to improve the quality of life for Phoenix residents and further the mission of the department are eligible to be nominated.

NOMINATING INSTRUCTIONS

• Individuals or groups may nominate more than one person or group, including themselves or their group.

• For a nominee to be considered, the nomination form must be filled out completely.

• Nominators must provide specific details when describing the nominee and focus on successful volunteer efforts, contributions and accomplishments.

• Nominations must be received by 4 p.m. Oct. 10, at the City of Phoenix Human Services Department, Attn. Lisa Araujo, 200 W. Washington St., 17th Floor, Phoenix, AZ 85003-1611.
