2008 Community Recognition Awards

The Human Services Department provides comprehensive social services to improve the quality of life for Phoenix residents.

Early in the Fall of 2008, nominations will be accepted by the city of Phoenix Human Services Department for the Community Recognition Awards Breakfast to be held in December 2008.

ELIGIBILITY

Any individuals, organizations or agencies who have contributed outstanding volunteer leadership or service to improve the quality of life for residents of Phoenix and further the mission of the department are eligible to be nominated.

NOMINATING PROCEDURES

• Individuals or groups may nominate more than one person or group, including themselves or their group.

• For a nominee to be considered, the nomination form must be filled out completely and all questions answered thoroughly.

• Nominators may attach additional pages if needed to provide complete responses to all questions listed on the nomination form. Continuation pages should include the nominee's name and restate the entire question(s) for which responses are continued.

• Nominators must provide specific details when describing the nominee and focus on successful volunteer efforts, contributions and accomplishments.

• During the nominating period, nominations should be submitted to: City of Phoenix Human Services Department, Community Recognition Awards Committee, Attn. Carol Montes, 200 W. Washington St., 17th Floor, Phoenix, AZ 85003-1611.
