To qualify for funding,
an applicant organization must meet the following criteria at the time
of application:
It must be a non-profit arts
or cultural organization based within the City of Phoenix.
it must have tax
exempt status under Section 501(c)3 of the Internal Revenue Code. Documentation
of this status must accompany the application.
It must submit IRS Form 990
"Return of Organization Exempt from Income Tax" for the past fiscal
year. (If the organization has an audited or reviewed financial
statement for the prior year, that should be submitted instead of the 990).
It must have been in existence
and actively programming for a minimum of three years.
It must have a minimum prior
year income of no less than $25,000
It must employ at least one
part-time paid staff member at the time of initial application.
