Fire Prevention Forms and Applications

Necessary steps to obtain a permit are available.  Next to most permit applications is a checklist with the required steps for your use.  Please complete the checklist and submit with your permit application.

General & Operating - For most general and operating permits complete the checklist and application, provide a copy of the Certificatate of Occupancy (CofO), if required, pay the fee and an inspector will call to set up an inspection. 

New Construction - An Admin Prelog Form is required for all permits that requires a plan review.  This document indicates everything that is required for a complete submittal.  

A Certificate of Occupancy (CofO) is required for all new construction permits and some general and operating permits.  Click here  to obtain a copy of the CofO.

If you'd like to pay with a credit card please complete the credit card form and fax it with your application, otherwise check or cash can be taken at the Fire Prevention office.
