Returning Completed Applications

Unless instructed to bring the completed application to the test site, completed applications are to be returned to the City of Phoenix Personnel Department, Application Office, 135 N. Second Ave., Phoenix, AZ, 85003. (Located 1 block South of Van Buren Street, Southeast corner of Monroe and 2nd Avenue)

This may be done by mail or in person or in the 24 hour drop box at the entrance to the Personnel Building. Applications must be received by the posted closing date.

If the application includes a related supplement, both must be returned by the closing date.
