Friday, May 9, is the last day to request an early ballot by mail for the May 20 special election on a ballot measure that would extend the existing sales tax for parks and preserves that is scheduled to expire in 2009.
Request forms to vote early by mail are available at phoenix.gov and at city public service counters and libraries.
Request forms also can be obtained by calling the City Clerk Department's Elections Division at 602-261-8683.
The City Clerk Department must receive signed written requests for an early ballot by mail no later than 5 p.m. Friday, May 9.
Voters may cast an early ballot or exchange a damaged or spoiled early ballot received by mail at early voting sites throughout the city until May 16.
