How To Apply For The AFP Program

Step #1

Request an AFP registration packet. You must be an official representative of a business or facility qualified and interested in participating in the Annual Facilities Program.

Step #2

Please read the entire registration packet, fill out the registration forms and provide the required signatures. Once completed, forward the forms along with this document and the appropriate registration fee to:

Step #3

Upon receiving your completed registration packet, staff will contact you to further discuss program services and address any additional questions you may have.
