How do I obtain a License?
The State and City applications for bingo in the city of Phoenix must be submitted directly to the city of Phoenix, City Clerk Department, License Services Office located at 200 W. Washington St., first floor, Phoenix, AZ 85003. The city of Phoenix makes a recommendation to the Arizona Department of Revenue (ADOR) on whether a License for a location in Phoenix should be granted or not. *The Arizona Department of Revenue issues all bingo licenses. Once the city of Phoenix completes the application review/approval, the ADOR requires additional review/approval time for bingo licenses.
