The City of Phoenix offers all full-time employees three City paid life insurance plans - Basic Life AD&D, Occupational AD&D, and Commuter Life insurance through Securian - Minnesota Life. In addition to, employees can elect for Optional Life Insurance for themselves, their spouse, and/or their eligible children.

City Group Life Insurance Options:
Basic Life and Accidental Death and Dismemberment Coverage
- Provided to full-time City employees at zero costs
- Coverage amounts are based on employee bargaining unit
- Coverage exceeding $50,000, IRS tax rules apply
- Basic AD&D matches the Basic Life coverage amount and follows a benefit schedule for dismemberment. Includes additional benefits for Felonious Assault, Bereavement and Trauma Counseling, Permanent Disfigurement/Critically Burned, Seatbelt, Coma, and Airbag.
Optional Life Insurance
- Coverage is provided at group rates for you, your spouse or qualified domestic partner, and/or your children.
- You pay 100% of the group premium with after-tax earnings through payroll deduction.
- Similar to an individual life insurance policy, this coverage may be subject to underwriting.

Occupational Accidental Death and Dismemberment
- Provided to full-time City employees at no cost.
- This coverage is payable when a death or covered accident occurs in the course of performing your job duties.
- This coverage pays in addition to the Basic Life coverage, when applicable.
- Coverage amounts are based on employee bargaining unit.
Commuter Life Insurance
- Coverage in the event of death within a two-hour timeframe while commuting to and from your established work locations
- Police reservists are not eligible
